Mary Woodman, NP-PHC, Project Manager, Quinte Health Link
Mary Woodman is a Nurse Practitioner and the Project Manager for the Quinte Health Link (QHL). In collaboration with a small but strong and dedicated team using a primary care lead model, the QHL has provided coordinated care for 900 patients to date. Mary served on the original Coordinated Care Plan (CCP) working group in 2013 and currently serves on HQO’s Clinical Reference Panel. She was a strong advocate for improvements to the CCP template, sharing the updates with, and incorporating feedback from, her colleagues in the SouthEast LHIN Community of Practice. Mary has recently been appointed to the new Clinician Digital Health Council, which will support the renewal of Ontario’s Digital Health Strategy.

Liz Laird, R.N., BScN, CHPCN(C)
Liz Laird is a Palliative Pain and Symptom Management Consultant with the Southwest St Joseph’s Healthcare PPSMC Program. Liz works in the Grey and Bruce county area providing consultation, education and system development in palliative care. She is an active member of the HCC ACP CoP for several years and contributed to the development of a number of the Ontario specific resources along with active advocacy for increased knowledge and practice reflective of the Ontario legal framework.
