Specialized Community-Based Care (Intermediate Care)
Specialized community-based care refers to services for people with chronic illness that are provided through formalized links between primary and specialized care. Another term for specialized community-based care is intermediate care. The intent of this analysis was threefold: 1) to review the literature on intermediate care for chronic diseases; 2) to synthesize the evidence from four previous analyses by HQO on specialized community-based care on chronic obstructive pulmonary disease (COPD), heart failure, diabetes and chronic wounds; and 3) to examine the role of intermediate care within family practice. There is convincing evidence that intermediate care improves outcomes in patients with heart failure, COPD and diabetes.
For this body of research, our Ontario Health Technology Advisory Committee (OHTAC) is making recommendations in four parts. This first “release” focuses on heart failure and includes standards of care for heart failure clinics across Ontario. The draft evidence-based analysis and OHTAC recommendations are now posted for public comment for a period of 21 days. Please submit comments to OHTAC_Comments@hqontario.ca.
Additional research and recommendations related to specialized community-based care for other chronic diseases will be released for public comments in the future.
