Data sources

The hospital patient safety indicators are based on data collected by the Ministry of Health, which are reported to them by all hospitals in Ontario through the Self-Reporting Initiative (SRI).

If you have a question about reporting on hospital patient safety performance in Ontario’s health system, please email us at OH-HQO_SystemPerformance@ontariohealth.ca.
