Welcome to Health Quality Ontario’s patient safety public reporting website. This site contains information about patient safety in Ontario hospitals.
Ontario hospitals take the provision of safe patient care seriously, including reporting data on several key indicators publically. Patient safety reporting involves providing the public with updates on patient safety in hospitals, including reports on hospital-associated infections, surgical site infection prevention, hand hygiene compliance, Surgical Safety Checklist compliance and hospital mortality rates.
Building on the Ministry of Health and Long-Term Care’s (MOHLTC) patient safety indicator initiative, HQO will continue patient safety public reporting efforts by providing provincial and individual hospital information about patient safety indicators that are required for public reporting by the ministry. The public can access these rates and compare them to other hospitals in the province and the overall rate for Ontario.
