Implementation Guidance for the Enhanced Health Link Measures

Learning objectives:

  • Provide an update from the Health Link Measures and Implementation Task Group

  • Highlight changes made to the Health Link measures and technical definitions launched previously this year

  • Understand the implementation and data collection process using different technical modalities including CHRIS, SHIIP and manual data collection
