Transitions From Youth to Adult Health Care Services

This quality standard will address care for youth who are transitioning (moving) from pediatric to adult health care services. The scope will include all clinical populations, including youth with special health care needs (physical, developmental, and/or mental health). The scope will include all health care providers and all pediatric and adult settings relevant to transitions and transfers of care, which includes primary care, hospital care, specialist care, and social and community services. The quality standard will focus on the continuum of care from receiving pediatric health care services to preparing for a successful transfer to adult health care services.

Ontario Health is working in partnership with the Provincial Council for Maternal and Child Health to produce this quality standard.
