An important strategy for improving health care in the long-term care setting is measurement and feedback to health care providers engaged in improvement and innovation. This consent and information page is to inform you of how measurement of processes and outcomes of your personal practice in the long-term care setting will be conducted through the Institute for Clinical Evaluative Sciences (ICES) and Health Quality Ontario (HQO) and to offer you an opportunity to participate.
