Primary Care Practice Reports give family physicians and executive directors of Community Health Centres and Family Health Teams access to personalized and confidential information about their practice as well as regional and provincial data for comparison. The report also includes contextual information and change ideas to help drive quality improvement.

The primary care practice data is updated semi-annually and made available to new and existing users who sign up for this program.

The Primary Care Practice Report provides cross-sectional and longitudinal data on practice demographics and case mix, patterns of service use (e.g., emergency department visits, hospital admissions, and specialist referrals), chronic disease prevention and management, and the health status of the practice population. They provide information at practice, regional and provincial levels.

Health Quality Ontario worked with the Association of Ontario Health Centres and the Association of Family Health Teams of Ontario to develop the report for executive directors in Community Health Centres and Family Health Teams. Consenting executive directors will receive Primary Care Practice Reports with aggregate organizational-level data, but no individual physician-level data. To receive their version of the report, executive directors must provide their consent to Health Quality Ontario. When doing so, they must confirm that no physicians in their Community Health Centres or Family Health Teams objected to having the executive director receive the aggregate level report.

If you are a primary care physician or an executive director for a Family Health Team or Community Health Centre, you can receive a personalized Primary Care Practice Report by completing the consent form found above. Once consent has been provided, a Health Quality Ontario account will be created to allow access to the reports. If you already have a Health Quality Ontario account, please sign in and complete the consent form.

The data in the report is both confidential and secure. The Institute for Clinical Evaluative Sciences (ICES) is named as a prescribed entity under Section 45 of Ontario’s health privacy legislation, Personal Health Information Protection Act (PHIPA), 2004, which provides the legal authority for ICES to conduct research about practice patterns. Consent is required to have data released from ICES to Health Quality Ontario to generate the reports. Data is at the aggregate-level for the physician or organization, therefore no patient-level data is received or reported. Data will not be released except to those consenting to receive it.

No patient health data are stored at Health Quality Ontario. Provider data stored at Health Quality Ontario to prepare the reports are stored on a secure server with limited staff access. In addition, reports will only be available to a physician or executive director via Health Quality Ontario’s secure portal after they have validated their identity.
