Health Links - Improving Integrated Care for Patients with Multiple Conditions and Complex Needs

The Health Links approach fosters an environment that helps healthcare teams develop new and better ways to integrate care delivery for the people of Ontario who live with complex chronic illness. The Health Links approach is a collaborative endeavour of MOH, LHINs, HQO and Health Links teams. Health Quality Ontario contributes to the Health Links approach by:

  • Supporting data collection, timely reports and analysis
  • Leading systematic identification of emerging innovations and best practices
  • Accelerating progress through standardization of best practices across all Health Links
  • Supporting inter-Health Links sharing of lessons learned on regional or pan-provincial basis
  • Connecting LHIN Health Links leads with other relevant provincial quality initiatives
