Please use your current username and password for the Practice Report web portal to access your MPLTC report when it becomes available.
What happens next?
- HQO confirms your consent to receive the MPLTC Report to ICES.
- ICES links your CPSO number to data in health administrative databases.1
- ICES provides HQO with aggregate data for your LHIN and the province.
- HQO generates and updates your MPLTC Report.
- HQO notifies you that your report is ready for download.
If your email address changes, please log in to the Practice Report web portal to update your email address, or notify HQO at practicereport@ontariohealth.ca.
