COVID-19: Get the latest updates or take a self-assessment.
Results for the 2020/21 and 2021/22 fiscal years should be interpreted with caution as the COVID-19 pandemic may have affected hospital admissions and emergency department visits.
This is the percentage of new publicly funded home care patients, of all ages, who had an unplanned emergency department visit in 30 days after leaving hospital. A lower percentage of patients is better. Timely follow-up after hospital discharge can help prevent the return of home care patients to the emergency department.
