Quality Improvement in Long-Term Care

Ontario’s long-term care (LTC) sector is committed to a culture of ongoing quality improvement to meet the evolving, complex needs of the tens of thousands of residents living in long-term care homes across the province. Health Quality Ontario and the long-term care sector work together to spark improvement by connecting care teams, clinicians, residents and researchers. They share data and best practices and foster knowledge exchange that helps improve the experience and outcomes for residents.


The data on our long- term care public reporting web pages for the Median Time to Long-Term Care Home Placement indicator has been updated! You can now see trends over time for years 2012/13 to 2016/17 for Ontario and LHIN regions, as well as results stratified by the resident’s prior location (i.e., hospital or community).

Long-Term Care Key Initiatives

Through collaborative knowledge and experience, reports and resources, key initiatives have been created to help long-term care facilities build knowledge and make improvement, such as:

  • The Appropriate Prescribing Demonstration Project, an educational program to support best practices in prescribing. This project is conducted by the Ministry of Health and Long-Term Care and Ontario Medical Association in collaboration with Health Quality Ontario and the Centre for Effective Practice

  • Long-term Care Practice Reports, provide long-term care providers data comparing their practices to what is happening provincially

  • Looking for Balance, a health system report by Health Quality Ontario about antipsychotic medication use in Ontario long-term care homes

  • Choosing Wisely Canada, a campaign to help physicians and patients have conversations about unnecessary tests, treatments and procedures
